Is there a form I can have my cleaning lady sign (or any other guest) to prevent her from suing me in case my dogs bite her in my house? My dogs do not have a history of biting, but I would like to protect myself and my dogs from having to put them down in case they bite her while she is chasing them around with a vacuum cleaner.

Why not have her move the dogs into a room and close the door while she vacuums the rest of the house and then open the door, let the dogs in to the rest of the house while5 she vacuums in the room where the dogs were initially.
Write down on a piece of paper a description of the dogs, state that they haven't bitten anyone before, but if she antagonizes them with her activities they could become excited because of their fear. Tell her in the document that she is to avoid at all costs exciting them by her activities so that they might react and harm her.
Put a sentence in it says: "You agree to indemnify and hold me harmless for any injuries you sustain if you are harmed by my dogs. You also agree to insist that they not be put to sleep if they should harm me. "I am paying you $100? per day for your work which includes working with my dogs in the room.
